Full Membership

For veterans with SCI/D, including MS or ALS

Who is eligible?


  • Membership is available to veterans with a spinal cord injury or disorder who meet the National PVA eligibility requirements.
  • Veterans who are already PVA members may transfer their membership to the Mountain States Chapter.

Complete the appropriate form. Applications are reviewed by PVA National to confirm eligibility and ensure proper enrollment. Email transfer applications to info@mscpva.org.

Associate Membership

For family, caregivers, supporters & professionals

Who is eligible?


  • Associate membership is open to family members, caregivers, health care professionals, community partners and others who support PVA’s mission.

Why Join?


  • Associate members strengthen the chapter by participating in chapter activities, supporting advocacy efforts and volunteering their time.

How Do I Apply?


National PVA Volunteer Form

  • Complete the form and email it to info@mscpva.org. Associate membership begins by registering as a PVA volunteer through the national volunteer system.
  • Volunteers receive liability coverage while volunteering and are included in the national volunteer database.
  • Upon approval, a national volunteer identification number is issued.
