For veterans with SCI, MS or ALS

Who is eligible?

Membership is available to veterans with a spinal cord injury or disease who meet the National PVA eligibility requirements.
Veterans who are already PVA members may transfer their membership to the Mountain States Chapter.

Complete the appropriate form. Applications are reviewed in coordination with PVA National to confirm eligibility and ensure proper enrollment.

Associate Membership

For family, Caregivers, Supporters & Professionals

Who is eligible?

Associate membership is open to individuals who support PVA’s mission, including family members, caregivers, health care professionals, and community supporters.

Why Join?

Associate members play an important role in strengthening the chapter through engagement, advocacy, and volunteer involvement.

How Do I Apply?

National PVA Volunteer Form

Complete the form. Associate membership begins by registering as a PVA volunteer through the national volunteer system.
This process enrolls individuals in the national volunteer database, provides liability coverage while volunteering.
Upon approval, a national volunteer identification number is issued.
Once registered, individuals may select the Mountain States Chapter as their home chapter and be recognized as associate members.
